I don't think we do actually have a written guide, but basically, it works like this:
If a moderator sees someone breaking our rules, they will send that person an infraction. What this does is to send a PM to the user with a note from the moderator. The moderator should explain what rule was broken, why they are sending the infraction, etc.
A note also gets attached to the user's profile that the administrators can refer back to later if there are recurring issues with a certain user - that way we can see if there has been a pattern of behavior despite the moderators trying to handle things, etc.
There is also a "point" assigned to the user when the infraction is sent, but we don't use the points system here - it was designed so that you can do things like automatically banning anyone who gets x number of points or whatever, but we believe that stuff like that should be handled on a case-by-case basis, and we don't want the system auto-banning people, so that part is turned off here.
