This thread is for videos and pictures of activities that are unforgiving. If you make a mistake riding a 70 foot wave or at isle of man going 200mph on your bike...you're going to die.

We just took a class on risk assessment. It's like learning how to jump out of planes, driving or riding a motorcycle. It's not the most dangerous when you're first learning. It's when you become familiar with it to stop being comfortable but not seasoned enough to actually be experienced that you're most likely to make a mistake and pay.

According to his wikipedia page, he died, ironically enough, when his rope broke:
Dan Osman died November 23, 1998 at the age of 35 after his rope failed while performing a "controlled free-fall" jump from the Leaning Tower rock formation in Yosemite National Park.
Edit:
Here's a vid explaining what happened. He was doing free-fall jumps with a rope. He broke the record with a 1000 foot jump (), went back a couple days later to do another one, and the rope snapped. This vid is of the record jump, but not the fatal job.
