This is why it is important to use hub-centric adapters, with the proper lug bolts/nuts.

I'm guessing this is what happens when you use spacers and stock length lug nuts.

Looks like his "Proper Attire" didn't include proper training of putting a wheel on.

On a serious note, I'm glad nobody was hurt and sorry for the damage. How bad was it?

Could be worse, you could have backed your car into a giant pole causing $3k in damage. Much more embarrassing then having your wheel fall off.

A couple years back a Vortex member helped do my brakes on one of my cars. A week later I went on a cruise with a bunch of people up Mt. Rainier. Turns out he didn't loctite one of the bolts on my Wilwood caliper and it came off my rotor and began rubbing the inside of my wheel while cruising downhill at a high rate of speed. **** happens.

    aint sh*t man!
i've had this happen while going 70 on the freeway.
my car was lowered enough at the time that my wheel stayed in the wheel well (with like 45 degrees of neg. camber).
destroyed the inside of my wheel (f*ck 'em) and my week old rotors fml
